The most consequential Medicare choice most people face is not which Medigap plan to buy or which Medicare Advantage carrier to pick โ€” it is the fundamental decision between Original Medicare and Medicare Advantage (Part C). This choice shapes your provider access, cost structure, and flexibility for years to come.

This guide explains the key differences, the factors that push most people toward one option or the other, and includes an interactive selector to help you identify your starting point. Remember that Medicare decisions are deeply personal โ€” your health situation, finances, preferred doctors, and where you live all matter. Use this as a starting point, not a final answer.

Original Medicare: Parts A and B

Original Medicare is the federal program administered by the Centers for Medicare & Medicaid Services (CMS). Part A covers inpatient hospital care, skilled nursing facility stays, and some home health services. Part B covers outpatient care, preventive services, and medical equipment.

Original Medicare’s biggest advantage is nationwide freedom: you can see any doctor or hospital that accepts Medicare โ€” roughly 93% of all physicians โ€” without referrals or network restrictions. This is especially valuable if you travel frequently, spend time in multiple states, or have specialists you need to continue seeing.

The trade-off is cost structure. Original Medicare covers 80% of most Part B costs; you pay the remaining 20% with no cap. A serious illness can expose you to substantial out-of-pocket costs. Most people pair Original Medicare with a Medigap supplement (like Plan G) to cover that 20% gap and cap their spending.

Medicare Advantage: Part C

Medicare Advantage plans are offered by private insurance companies that contract with Medicare to provide all Part A and Part B benefits โ€” often bundled with Part D prescription drug coverage, plus extras like vision, dental, and hearing.

The main appeal of Medicare Advantage is lower or zero monthly premiums and the inclusion of extra benefits Original Medicare doesn’t cover. Many plans have $0 premiums (you still pay the Part B premium). These plans often also include telehealth, fitness memberships, and transportation benefits.

The trade-off is network restriction. Medicare Advantage plans are HMO or PPO networks. If you see out-of-network providers without authorization, you may pay significantly more or the entire cost. And if you move, travel frequently, or need specialist care in another region, your coverage may not follow you the same way Original Medicare does.

The Decision That’s Hard to Reverse

One of the most important things to know: switching from Medicare Advantage back to Original Medicare with full Medigap coverage is not guaranteed. In most states, Medigap insurers can use medical underwriting if you try to enroll outside of your initial enrollment window. If you have health conditions when you try to switch back, you may be declined or charged higher premiums.

This asymmetry means many advisors suggest that people who can afford the Medigap premium consider starting with Original Medicare + Medigap, since moving to Medicare Advantage later is generally easier than moving back.
